Name Change Temporarily on Hold

Dear Esteemed Family Member and Resident,
I am writing to introduce myself and to keep you apprised of the happenings in our facility. As you may have heard, Grace Healthcare of Three Rivers will undergo a change in ownership. It is our goal to continue to provide the best care to you and your loved ones, and to continue the focus on service the team at Grace Healthcare of Three Rivers has worked so diligently to develop.
We are a small family owned company, with a focus on quality and community. After meeting with members of the local team, we have decided to name our new center Edgewood Health and Rehabilitation. You will find that our philosophy of “operating locally” extends well beyond naming the center. Many of our decision will involve input from the staff and members of the community. We encourage you to join us in charting the future of Edgewood Health and Rehabilitation.
The direct caregivers you have come to know will remain with Edgewood Health and Rehabilitation and will continue to provide the excellent care and service you have come to expect. This transition may bring about some internal changes, although ultimately, we endeavor to make the changes for the betterment of our residents. Please drop off any correspondence (documentation, checks, etc.) directly to the center going forward.
To introduce myself personally: I have been in skilled nursing care for 25 years. My first job was as a social worker where I developed a love and passion for assisting residents and families. I believe that if we treat each other like family and genuinely care for residents, staff, and families, quality outcomes will always follow. We will always be transparent and available. Please feel free to contact me if you ever have questions which cannot be resolved locally in the community.
